Community Habilitation programs assist persons with developmental impairments in living freely in their communities. Participants in these programs work one-on-one with a Direct Support Professional, who assists them in selecting community-based activities and establishing personal objectives. They could also take part in group activities. Transportation is also provided by the programs. The community rehabilitation team also strives to enhance the participants' general quality of life. The services are offered in collaboration with the individual's family. Community Habilitation services are available in a number of locations, including certified residential homes and alternative arrangements. These services are person-centered, with the goal of supporting the valued objectives indicated in the Individualized Service Plan (ISP). They must have a valid New York State driver's license and evidence of personal insurance.
